If your enquiry is of a routine nature, you may complete the report form below. Reports submitted will be reviewed by animal helpline control staff however, it should not be used if the animal is in immediate danger.

Please ensure you have completed as much information as possible as it will allow us to investigate your concerns efficiently. The Inspector assigned to your report may contact you for further information. All information is provided in confidence and is held in accordance with the Data Protection Act.
If you have concern for an animal that is in immediate danger or is in distress you need to contact our animal helpline as we may have specific questions in relation to the incident which may lead to a delay in specialist resources responding.
